Community Care and Support Services for the Elderly

We provide support in household duties, personal care, nursing care, rehabilitation, day care, carer support, health promotion, and community support services to the elderly or families in need and persons with disability. These services enable elders to stay at home of their choice, while the carers are supported to perform their caring role.

Family-based Dementia Assessment and Support Project

This project will be ended on 31st March, 2022

This project aims at raising community understanding of dementia, awareness of modifiable risk factors; early identification of elderly with dementia symptoms and make referrals for early intervention(s); support for elderly with dementia and / or their caregivers to enhance their competence / reduce their stress in living with dementia.

Service targets

  • Aged 65 or above;
  • Elderly not yet diagnosed with dementia by any medical professional; and
  • Elderly identified with early signs of dementia.

Dementia Assessment Service

Service Introduction

Certified Dementia Care Planner or qualified professionals will conduct detailed assessment of persons with cognitive impairment symptoms with internationally recognized tools to facilitate early intervention.

Service Content

  • Professional assessment (one-off)
  • Detailed assessment report
  • Explaining the report to the elder/caregivers
  • Service referral (if needed)

 

Dementia Care Planning Service

Service Introduction

Certified Dementia Care Planner or qualified professionals will comprehensively assess the needs of caregivers, assist them in setting goals and systematically facing care challenges.

  • Caregivers of persons with dementia

Service Content

Understanding dementia, Adjustment to newly diagnosed dementia, Keeping person with dementia safe, Communication, Daily care, Challenge & Coping Strategies, Cognitive Training, Advance Care Planning, Navigating resources, others, etc.

 

Dementia Caregivers Training Group

Service Introduction

6 sessions led by Certified Dementia Care Planner or qualified professionals to enhance caregivers competence and reduce their stress.

Dementia Caregivers Support Group

Target Users

  • Caregivers of persons with dementia
