We provide support in household duties, personal care, nursing care, rehabilitation, day care, carers’ support, health promotion, and community support services to the elderly or families in need and persons with disability. These services enable elders to stay at home of their choice, while the carers are supported to perform their caring role.
Elderly Community Support Service provides different services for the elderly to facilitate them to live healthy, positive, active and happy lives. Integrated Home Care Services provides a wide spectrum of home care and support services by 8 Integrated Home Care (IHC) teams, e.g. nursing care, rehabilitation service, home safety and advice, home modification, personal care, escorting service, day respite, child care, laundry, meal delivery service, purchase of daily necessities, house cleaning, psycho-social intervention and referral service, etc. Day Care Service includes day care centre, home based rehabilitation and care, home care and care-giver support.
Home Support Services
To provide holistic services to the target service users living in the community to facilitate their continuous living at home for as long as possible and achieve or maintain their optimal level of functioning; and to offer support services for carers to alleviate their care stress. To provide appropriate services for the elderly, people with disabilities, and individuals and families with social needs, in order to actualize the concepts of ‘aging in place’ and ‘continuum of care’.
Target Users
People aged 60 or above who are assessed to be at the state of mild impairment or requiring higher level of care under interRAI™ Check-up assessment tool or assessed and recommended for Home Support Services under the Standardised Care Need Assessment Mechanism for Elderly Services.
People with disabilities or individuals and families with social needs (Have to be referred by registered social workers from social welfare service units. For individual with mental illness or infectious disease, certificate from medical doctor is required to demonstrate stable condition).
Service Delivery
Provided a wide spectrum of home care and support services by Home Support Services (HSS) teams, e.g. Personal care, Household cleaning, Transportation and escort service, Home respite, General physical exercise, Environment risk assessment and home modifications suggestions, Meal service, Basic health care, Carer support services and carer training, Counselling, Other services, e.g. Arrangement or referral for centre-based / residential respite services, Care management and assessment.
Service Hours
Monday to Saturday: 9:00am to 5:30pm
On Sundays and public holidays, suitable service arrangement will be provided according to the needs of service users after assessment.
